Dermal Fillers
Volume is the part of facial ageing people notice last and misdiagnose most. The face does not only sag; it deflates.

01 · Understanding the concern
Why patients consider this
With age, facial fat compartments shrink and bone remodels, particularly around the mid-face and the eye socket. The result is flattening of the cheek, hollowing under the eye and deepening of the folds that run from nose to mouth. Treating that as though it were excess skin leads to the wrong intervention.
02 · The procedure
What dermal fillers involves
Hyaluronic acid fillers are gels of varying firmness placed at specific depths — deep on bone for structural support, more superficially for fine contour. Product choice, plane and volume all follow the anatomical problem. Treatment aims to restore support in the right place rather than to fill the visible line directly, which is what avoids a heavy, overfilled look.
03 · Candidacy
Who may be considered
This is general guidance rather than a diagnosis. Suitability is decided by examination.
- Mid-face flattening or volume loss
- Hollowing under the eyes, where the anatomy suits treatment
- Deepening folds around the mouth
- No active skin infection at the site
- Not pregnant or breastfeeding
What it can address
Concerns it is used for
- Cheek and mid-face volume
- Tear trough hollowing, in selected cases
- Nasolabial folds
- Marionette lines
- Jawline and chin contour
- Temple hollowing

04 · Planning
Why the planning matters more than the technique
Filler restores volume; it does not lift descended tissue. Where descent is the dominant problem, adding volume makes the face heavier rather than younger. Recognising which patients have reached that point, and saying so, is part of the consultation.
05 · How it works
Step by step
- 01
Assessment
Facial analysis distinguishing volume loss from descent and skin change.
- 02
Planning
Product, plane and volume selected per area.
- 03
Anaesthesia
Topical anaesthetic; most products contain local anaesthetic.
- 04
Placement
Product placed by needle or cannula according to the area and depth.
- 05
Assessment
Symmetry and contour checked before you leave.
- 06
Review
Reassessment once swelling settles, with adjustment if required.
06 · The procedure journey
Before, during and after
Before
Avoid blood-thinning supplements where advised. Report any history of cold sores or autoimmune conditions.
During
A clinic appointment, generally under an hour depending on the areas treated.
After
Swelling and occasional bruising for several days, with the area looking fuller than the settled result.
07 · Recovery
What recovery generally involves
No meaningful downtime. Exercise, heat and alcohol are avoided briefly. Results are judged at around two weeks.
Recovery times are general ranges. Your own timeline depends on the extent of treatment, your health and your work, and is given at consultation.
- Avoid heat, sauna and heavy exercise for 24 to 48 hours
- Do not massage treated areas unless instructed
- Sleep slightly elevated the first night
- Assess at two weeks
- Report severe pain, blanching or vision change immediately
08 · Outcomes
What to expect from the result
Hyaluronic acid fillers typically last from several months to over a year depending on product and area. Results vary between individuals and are not guaranteed.
09 · Safety
Risks and considerations
Risks include swelling, bruising, lumpiness, asymmetry, delayed nodules, and rarely vascular occlusion which can threaten skin or vision and requires immediate treatment. Anatomical knowledge, product choice and the ability to manage complications are why this is performed by a qualified practitioner rather than in a salon setting.
No procedure is risk-free. A full, individual discussion of risk forms part of consent and takes place before any decision is made.

Questions patients ask
Dermal Fillers: common questions
How long do fillers last?
Commonly several months to over a year depending on the product, the area treated and individual metabolism.
Can fillers be dissolved?
Hyaluronic acid fillers can generally be dissolved with an enzyme if the result is not wanted or in the event of a complication.
Will I look overfilled?
Not if volume is placed where it is structurally needed rather than into visible lines. Conservative, staged treatment is the approach used here.
Are under-eye fillers safe?
The tear trough is a technically demanding area and not suitable for everyone. Candidacy is assessed carefully.
Filler or fat transfer?
Fillers are adjustable and reversible; fat is your own tissue and lasts longer for the surviving portion. The choice depends on your goals.
Can I combine fillers with Botox?
Frequently. They address different problems and are often planned together.
